Hepatic artery aneurysms are a rare cause of upper gastrointestinal hemorrhage
and may represent significant problems in both diagnosis and management. We
report a 70 year old patient with gastrointestinal bleeding from a fistula between
a 7 cm hepatic artery aneurysm and the duodenum. He underwent successful
surgical management with endoaneurysmorrhaphy and duodenal wall repair.

DIPHALLUS: A REPORT OF AN UNUSUAL CASE

We report an unusual case of diphaIlus in a five year old boy. The two organs
appeared separately, one orthotopically in the pubic area and the other in the
perineum with no urinary meatus. A perineal hypospadias with meatus in the
anterior anal verge was also present. The right testis was undescended and the
scrotum was mildly bifid. The patient was continent for both voiding and
defecation. Micturition was via the hypospadiac perineal meatus under voluntary
control. A review of published cases suggests that this may be the first example of
complete diphallia with two separate phalli in different locations.

SPLENIC HAMARTOMA: A CASE REPORT

A rare case of splenic harmartoma in a 31 year old female is reported. She was
admitted with splenomegaly, fever and chronic history of epistaxis. Routine
hematological survey revealed pancytopenia and mild hypercellular marrow.
Abdominal sonography showed a space-occupying lesion in the spleen which was
suspected as malignant tumor. Splenectomy was performed and histologic findings
were diagnosed as hamartoma. The epistaxis and pancytopenia disappeared
two months after splenectomy, and at present the patient is doing well.

Zygomycosis is a fulminant infection which usually afflicts patients who are
immunocompromised by diabetes mellitus, malignancy or immunosuppressive
therapy.l This disease is a rare but highly invasive fungal infection which occurs
in organ transplant recipients. 2 Mucormycosis confined to the gastrointestinal tract
is an uncommon form of the disease. accounting for only seven percent of all
mucormycoses.3 ..c We are reporting a case of gastrointestinal mucormycosis in a
young woman, perhaps for the first time in Iran.

Patent ductus arteriosus (PDA) is an open communication between the upper
descending thoracic aorta and the proximal left pulmonary artery, which results
from persistence of the fetal ductus arteriosus. It may be isolated or occur with
other congenital cardiac anomalies.
Aneurysms of the ductus arteriosus are rare lesions which may occur as a
spontaneous. infantile fonn or occur in later childhood oradult life. About60cases
of ductus aneurysm have been reported in infants less than two months old, and
only about 40 cases have been reported so far in adults in the world literature. In
this report, a case of huge aneurysm of the ductus arteriosus in a five year old child
associated with ventricular septal defect and pulmonary stenosis is presented, the
first case of this kind in the literature, Successful surgical correction was performed.

As part of a series of investigations at Baba Baghi Leprosarium in lran, 44
long-treated leprosy patients were selected for our study. Samples of early
morning sputum were obtained from each patient, examined by microscopy for
acid-fast bacilli (AFB), and cultured for tubercle bacilli. These tests were negative,
but the polymerase chain reaction (PCR) for an insertion sequence believed to be
specific for Mycobacterium tuberculosis was applied to each sputum sample and
those from six patients were found to be positive. Five of the six positive samples
were from the 21 patients producing Koch-type responses to tuberculin, and none
were from the 11 patients previously found to have skin-tissue fluid or sputum
positive by PCR for Mycobacterium leprae. Whereas immunotherapy with killed
Mycobacterium vaccae given nearly 2 years earlier to 23 of the patients strongly
influenced PCR results for M. leprae (p= 0.0 I), it had no influence on results fm'
tubercle bacilli. However, at a second sampling date 18 months later, the only 2
patients still positive by PCR for tubercle bacilli came from the placebo recipient
group. The possible significance of the findings is discussed.

INTESTINAL ZINC ABSORPTION IN DESFERROXAMINE-TREATED RATS

Desferroxamine (Desferal) is used tochelate iron in thalassemia, a disease of
very high prevalence in various parts of lran. We have investigated intestinal zinc
absorption in Desferal-treated rats which, in pilot experiments, showed low serum
levels of both iron and zinc. Intraperitoneal administration of ten 0.5 g doses of
Desferal (one injection every other day) to male rats produced severe iron
deficiency accompanied by lower plasma zinc levels. An oral dose of 0.4 mg zinc
sulfate, given to Desferal-treated rats, resulted in a higher increase in mucosal zinc
absorption after 2 hours compared to normal controls. Similar results were
obtained when the oral dose contained 0.4 mg each of ferrous sulfate and zinc
sulfate. This suggests that the presence of iron has no effect on intestinal zinc
absorption with the dose employed in this study. Zinc absorption increased with
an oral zinc dose given to normal rats while iron absorption did not increase with
an oral iron plus zinc dose given to normal rats, suggesting the absence of a
regulatory mechanism for zinc absorption, similar to the one known to exist for
iron absorption.

This study was performed in order to define the cellular immune response of
12 recovered cutaneous leishmaniasis subjects to different soluble antigens of the
amastigote form of Leishmania major. A soluble leishmanial antigen preparation
(SLA) derived from highly-purified amastigotes from infected nude mice was
fractionated by Mono Q column using Fast Protein Liquid Chromatography
(FPLC) system. Three different fractions were obtained. The lymphoproliferative
response, interferon•gamma (IFN.'Y) and interleukin•4 (JL•4) to amastigote SLA
and its three subfractions were measured. The highest proliferative response and
specific IFN.'Y production without synthesis of fL•4, was induced by the fITst
fraction of amastigote SLA. These results showed that the individuals who had
recovered from cutaneous leishmaniasis had expanded memory T- cell clones
recognizing different antigens in the first fraction of amastigote SLA.
The main and most important point of this study was the identification of one
fraction of an amastigote antigen which preferentially reacted with cells from
recovered human cutaneous leishmaniasis cases.

The second somatosensory area (SlI) has been studied both by electrical
evoked potentials24 and magnetoencephalography (MEG)5 Magnetic evoked
fields of contralateral primary somatosensory and ipsilateral second somatosensory
cortices of 12 normal subjects were recorded in response to median nerve electrical
stimulation by means of a single magnetometer. We detected. in addition to the
usual contralateral SI responses, ipsilateral second somatosensory evoked fields
(sSEF's) in 4 subjects at a latency of 35-45 ms over the dominant hemisphere for
handedness, i.e., right hemisphere in left handed subjects and vice versa. On no
occasion were ipsilateral somatosensory fields recorded from the non-dominant
hemisphere whichever median nerve was stimulated. There were no consistent
responses at 90-150 ms. We concluded that the ipsilateral responses were from the
SlI area.
Our observations indicate the following: a) despite the known bilateral
representation of SII, there is hemispheric dominance, b) we could not confirm the
long latency (>95 ms) signals reported by Hari et al. in any of our subjects.5.6

THE EFFECT OF MECHLOROETHAMIN ON RECONSTITUTED CHROMATIN

In this study the effect of a methylated derivative of nitrogen mustard
(mechloroethamin) on solubilized and reconstituted chromatin was investigated
using UVNis spectroscopy, DNA and SDS gel electrophoresis and densitomeoic
scanning techniques,
The results indicated that the 210 and 260 nm absorbances of reconstituted
chromatin were decreased in a dose-dependent manner after treatment with various
concentrations of mechloroethamin. Alkylated chromatin showed difficulty in
producing nucleosomes in comparison to the control, therefore a 50% reduction in
absorbances was obtained. The amount of both core histone protein and DNA in the
supernatant was decreased with increasing drug concentrations.
It is suggested that mechloroethamin alters the interaction of ON A protein in the
chromatin by producing cross-links between DNA and protein or protein and protein.

Serum alpha-tocopherol of98(62 male and 36 female) cancer patients and 30
(13 male and 17 female) control healthy subjects was investigated. The cancer
patients showed a significantly low level of alpha-tocopherol as compared to
control healthy subjects. Also, significantly higher levels of serum alpha-tocopherol
were found in complete remission patients as compared to the partial
response patients.

Antineutrophil cytoplasmic autoantibodies (ANCA) were first described in
patients with necrotizing glomerulonephritis. The original observation passed
unnoticed until an association was made between ANCA and active Wegener's
granulomatosis. Since then, tremendous progress has been made in elucidating the
association between ANCA subtypes and clinicopathologic syndromes, and the
potential pathologic role of ANCA in vascular inflammation. The gold standard
method for ANCA detection is indirect immunofluorescence (IIF) microscopy of
ethanol-fixed cells. By this technique two general patterns of ANCA, acytoplasmic
pattern (c-ANCA) and a perinuclear pattern (P-ANCA) can be identifed. Recently
some reports have been published concerning the prevalence, subtypes, role and
specificity of ANCA.ln this study we searched for ANCA prevalence, subtypes,
and its relationship with disease activity in SLE patients by IIF technique. Our
results showed that all normal subjects were ANCA negative, while at least 50%
of patients were ANCA positive (majority c-ANCA type). Statistical analysis
revealed that there is no correlation between the presence of ANCA and disease
activity (p<0.01).

CLINICAL PRESENTATION OF GLUCOSE-6- PHOSPHATE DEHYDROGENA SE (G6PD) DEFICIENCY: A PILOT STUDY

Sixty-six children with G6PD deficiency were evaluated retrospectively to
ascertain the clinical features, etiology, ultimate outcome and population at risk,
The occurrence of jaundice in 18 neonates (group J) was, contrary to other
countries, in the form of neonatal jaundice type II. Sepsis, prematurity, hypoxia
and acidosis were associating factors. 77.8% of neonates had exchange blood
transfusions and 22% had kernicterus.
The occurrence of acute hemolytic anemia in 48 children (group II) was seen
at 3.4±1.6 years of age and the factor initiating hemolysis in the majority of cases
was fava bean ingestion. 79% of group 0 had at least one blood transfusion for
severe anemia. Chronic anemia in 6.2% suggested congenital nonspherocytic
hemolytic anemia.
Most patients had blood group 0, thus showing a higher prevalence than the
general population. Among children, the birth places in 22 cases were in the north
(77.2%), the west (18.1 %), and other pans (9.7%) of lran. 30.4% of children had
other family members with G6PD deficiency.
According to WHO and considering the prevalence of the disease in our
country, lab screening tests are strongly recommended in the following situations:
neonates with jaundice, males having blood group O. a family history of the
disease and children from the north and west of Iran.

ADJUSTABLE SUTURE IN STRABISMUS SURGERY

Adjustable suture in strabismus surgery is a way to get accurate alignment of
the eyes after operation. This technique is used in cooperative patients, being either
adults or children.
We report the result of this technique in 120 patients, 70 with esotropia and
50 exotropia. The amount of deviation was between 20 and 60 prismdiopters. The
patients were mostly adults and teenagers. and the youngest patient was 10 years
old. Post-adjustment deviation was less than 15 prism diopters in almost all cases.
After 2 to 3 years of follow-up, no patient needed reoperation.

We have used recombinant human erythropoietin (r-HuEPO) in children with
chronic renal failure. There was significant improvement in anemia and wellbeing,
along with regression of left ventricular mass and no change in the rate of
decline of renal function. It is safe and effective in the pre-dialysis as well as the
hemodialysis period. Considering the benefits of r-HuEPO, transplantation in
children can be postponed to a later time, when sufficient growth and development
have occurred, leading to better results of transplantation
