ALUMINUM TOXICITY: A REVIEW IN RELATION TO CHRONIC RENAL FAILURE PATIENTS M AINTAINED ON REGULAR HEMODIALYSIS

Aluminum is present in very small amounts in living organisms but abundant
in the environment. A growing literature links with the biochemistry of aluminum
and also with a series of diseases in chronic renal failure patients on treatment with
hemodialysis. The initial description of potential aluminum toxicity in renal
failure patients relates to description of dialysis encephalopathy syndrome in
1972. The major emphasise of this review will be on the recent literature involving
aluminum metabolism and epidemiology of aluminum related disease. Finally
the possibility that aluminum contributes to hypochromic microcytic anemia,
dialysis osteomalacia (Newcastle bone disease), encephalopathy and Alzheimer
disease in hemodialyzed patients has been also discussed.

Ellis-van Creveld syndrome is transmitted as an autosomal recessive trait.
This syndrome is accompanied in 60% of cases with congenital heart disease,
mostly single atrium or large ASD. Patients are mostly symptomatic, but in this rare
case despite 68 years of age, the patient was free of symptoms except for complete
heart block for which pacemaker was inserted

A three year old boy with delayed separation of the umbilical cord, recurrent
pyogenic infections and common variable immundeficiency is presented. The
immunoglobulin measurement was severely abnormal. To the best of our knowledge,
the existance of leukocyte adhesion disorder together with common variable
immunodeficiency (CYID) in a patient has not been reported prior to this study.

Eosinophilic gastroenteritis is a rarely reported condition. The disease is
characterized by peripheral blood eosinophilia, eosinophilic infiltration of the
bowel wall to a variable depth, and gastrointestinal complaints. Allergic diathesis
can be found in most of the cases. In this article we report two cases with interesting
epidemiological characteristics and we will have a brief review of reported cases
in the English literature

STU DIES ON THE BINDING OF THE ALKYLATING AGENT SULFUR MUSTARD TO CALF THYMUS CHROMATIN

In this study the effect of the alkylating agent, sulfur mustard, on calf thymus
chromatin was investigated using UV/Vis spectroscopy, gel electrophoresis and
thermal denaturation techniques. The results show that treatment of isolated
chromatin with sulfur mustard releases histones from the core particles but does
not affect histone H I and nonhistone chromosomal proteins. The content of
released proteins for the amounts lower than 50µg/mL are both dose and time
dependent. It is suggested that they have cross linked reaction to other chromosomal
proteins or to DNA. Thermal melting analysis also indicate that the
interaction of sulfur mustard with DNA increases thermal melt(Tm) of DNA
revealing condensation of DNA molecule by sulfur mustard. The results in relation
to the functional states of chromatin are discussed.

SCREENING FOR STARCH-HYDROLYSING BACTERIA

Screening of3000 soil samples collected from cities of four different provinces of
Iran for starch-hydrolysing bacteria revealed that the nature is enriched with
Streptomyces species capable of producing amylolytic enzymes. Among the bacterial
isolates, one of the high starch-degrading strains was selected for further microbiological
identification and also amylolytic enzyme(s) characterization. The purified
isolate, Streptomyces species strain R Y 48, produces almost four times more amylolytic
enzyme(s) than Bacilllls slIbtilis (PTCC 1254) on agar plates. Based on the action
pattern of its amylolytic enzymes on the boiled maize starch, the purified species
possesses in the logphase an amylolytic activity which is different from the amylolytic
activity of the stationary phase. The secreted amylolytic enzymes which are both
endolytic type and show different activity bands on native polyacrylamide gel, act
independently on starch molecules.

Incubation of rat hepatocytes with epinephrine inhibited alanine
aminotransferase (ALT) (80%) and aspartate aminotransferase (AST) (53%)
activities with no effect on lactate dehydrogenase (LDH) activity. Injection of
epinephrine caused a progressive increase with time in hepatic LDH activity, being
52% at 24 h. Preinjection with propranolol eliminated the hormone effect and
caused further inhibition (28%) of the enzyme activity. Liver ALT activity in
ephinephrine-treated animals decreased by 37% at 24 h, "_.er which it rose again
to the control levels at 48 h. Propranolol raised epinephrine-induced enzyme
inhibition up to 53%. Phentolamine, however, abolished the inhibitory effect. The
activity of hepatic AST was not affected by epinephrine. Plasma ALT and LDH
activity increased in epinephrine injected rats by 55% and 75%, respectively, but
AST activity did not change. Propranolol alone stimulated serum ALT (41 %) and
AST (21 %) activities. The data suggest that epinephrine exerts its effects on these
enzymes through a-receptor activation and/or via the cell lesion resulting in the
change of intra- and extracellular enzyme levels.

Cell-mediated immunity (CMI) plays an important role in resistance against
leishmaniasis. Delayed-type hypersensitivity (DTH) reaction measured by skin
testing is a practical method of evaluation of CMI and is used as an aid to diagnosis
and for epidemiological assessment of exposure to leishmanial infection. Skin
testing in leishmaniasis, generally known as Montenegro or leishmanin test, requires
a standard antigen. At present no uniform and standard leishmanin is available for
skin testing in leishmaniasis. The present work describes the preparation and testing
of an antigen from Leishmania major using standard conditions. Three dilutions of
this antigen were tested in recovered individuals in an endemic area in Iran. The
results obtained showed that leishmanin preparation exhibited high specificity and
sensitivity, and strong potency.

ISOLATED Z YGOMATICO-ORBI TAL INJURIES (lZOIs) IN TEHRAN: AN ANALYSIS OF 375 C ASES

Zygomatico-orbital injuries (ZOls) are the most common injuries treated by the
oral and maxillofacial surgeons. A respective analysis of 375 pateints with 1Z0ls
treated in Najmeia and Baqiet-Ullah University Hospitals during a six-year period
from 1984 to 1990 is presented. Male cases accounted for 94.4% (354) 59.46% (223)
of the fractures involved the zygomatic bone, 70.13% (259) were fractures due to
gunshot injuries, and 29.33% (I 10) were due to motor vehicle accidents (MV A). The
etiology, pattern, and incidence of the injuries, as well as age, sex, presence of
infection, time of treatments, and the results are discussed.

ETIOLOGIC AGENTS AND DRUG SENSITI VITY IN URINARY TRACT INFECTIONS: A SURVEY OF 786 URINE CULTURES

In this survey, 786 urine cultures were studied retrospectively at Shaheed Dr.
Rahnamoon Hospital in Tehran. The study showed that E. coli is the most common
causative agent of urinary tract infections in both sexes, being responsible for 72%
of all cases. Other etiologic microorganisms, in order of frequency, were Staphylococcus,
Proteus, Klebsiella, Enterobacter and diphteroids. Approximately 9% of
all causative microorganisms were partially or totally resistant to the antimicrobials
used in the antibiograms and practically all were among the nosocomial
infections. The sensitivity/resistance ratio of the etiologic agents in this study
revealed that E. coli was most sensitive to amikacin, nitrofurantoin and nalidixic
acid. However, it was resistant to ampicillin in 90% of all cases (compared to only
30% resistance rate in the western countries), which is probably due to inappropriate
use of antibiotics in our country. We therefore conclude that the administration
of ampicillin and co-trimoxazole, the two antimicrobials frequently used as an
impirical therapy for urinary tract infections, is not appropriate.

EARLY AND DELAYED RESULTS OF LUMBAR DISC SURGERY IN 314 PATIENTS

In a retrospective study the clinical presentations, neurological findings, contrast
studies, operative findings, and the follow-up results of 314 patients with disc
herniation were studied over a 10-year period. This disease was most frequent
between the ages of 30 to 40 and mostly in males. Low back pain with or without
radicular pain was the most frequent symptom, involving the left leg in the majority
of patients and the average duration of pain was more than a year. In this study
L4 -L5 disc herniation had the highest frequency (50%). The most encouraging sign
to perform myelography was motor deficit (57%). Postoperatively, our patients were
followed for an average of 12 months with excellent to good results noted in 96% of
them. Reherniation of a previously resected disc was noted only in two patients
(0.6%). It is concluded that with precise selection wecould have more than 90% good
results in a conventional method of disc surgery and the chances of recurrence are
negligible.

ANATOMICAL VARIATIONS OF THE CHORDA TYMPANI NERVE

The author studied anatomical variations of the origin of the chorda tympani
nerve from the mastoid portion of the facial nerve. The studies were performed on
200 specimens of human temporal bone. It was found that the chorda tympani may
arise from the facial nerve in three levels: most commonly from the mid third of its
mastoid segment (70%), less commonly from the proximal third (20%), and very
rerly from the distal third (10%). Knowledge of these variations has a certain value
in surgical intervention of the middle ear.

EFFECTS OF CIS APRIDE ON CONS TIPATION DUE TO SPINAL CORD INJURY: REPORT OF FIVE WARFARE SPINAL CORD INJURED PATIENTS

Five warfare spinal cord injured patients with intractable constipation are
described. Treatment with cisapride (4 x 10 mg daily) was undertaken. The agent
cisapride significantly reduced the oral-anal transit time from 25.2 days to 14.4 days.
It also improved other subjective complaints of these patients markedly. No side
effects were seen during the trial.

From October 1990, to June 1992, more than 800 electroejaculations and
108 intrauterine inseminations (IUI) were performed in 250 paraplegic males
and their wives. Seven successful pregnancies are the results of this 20-month
effort. This is the first report of electroejaculation and successful pregnancy
with its use in paraplegics in Iran and the Middle East.
