MALIGNANT HISTIOCYTOSIS: A CASE REPORT AND REVIEW OF THE LITERATURE
Malignant histiocytosis (MH) is a rare hematologic malignancy, especially in
the first decade of life. The disease is clinically characterized by fever,
hepatosplenomegaly, lymphadenopathy, pancytopenia and jaundice, and
histologically by systemic proliferation of malignant histiocytes and
hemophagocytosis. The prognosis is poor and often the diagnosis is not made
before death. Because of the rarity of this disease, it is unusual for practitioners to
diagnose it by bone marrow aspiration (BMA) alone

PREPARATION AN D CLINICAL EVALUATION OF SOME ANTI-ANORECTAL DISEASE DRUGS OBTAINED FROM MEDICINAL PLANTS
The hydroalcoholic extracts of dried ligulate florets of Calendula officinalis
L., dried leaves of Plantago major L., and dried aerial parts of Vinca major L. were
prepared and used as the main active ingredients of two types of anti-anorectal
disease ointments and suppositories. Benzocaine was used in one of the
formulations, while a mixture of belladonna dry extract and ephedrine sulfate was
used in the other. The effects of both prepared formulations were investigated in
patients suffering from anorectal diseases, using relevant placebos for
comparison.
The results indicated that the prepared drugs were significantly effective in the
treatment of anorectal diseases. Formulations containing benzocaine proved to be
more effective in patients suffering from hemorrhoids, while formulations containing
belladonna dry extract and ephedrine sulfate were more effective in anal
fissure and proctitis.

MONITORING OF LEAD POISONING IN SIMPLE WORKERS OF A COPYING CENTER BY FLAME ATOMIC ABSORPTION SPECTROSCOPY
Inorganic lead compounds are used most widely in paint and pigment
industries. Because of exposure to lead dust in workers in a copying center, we
compared blood lead levels in these workers to normal individuals and investigated
the relationship between job tenures of the workers and their blood lead levels.
This survey was performed on 20 simple workers in a copying center of Tehran
University. The ages of the workers were between 27 and 62 years, and job tenures
of the workers were between I and 27 years. Our results indicated that 94% of the
workers had high blood lead concentrations (>49.9 µg/dl) and only 6% showed
acceptable (30-49.9 µg/dl) blood lead levels. Blood lead levels of these workers
were significantly higher than that of controls (P<0.01). Their blood lead levels
and job tenures showed a direct but imperfect correlation [coefficient of correlation
(r)=0.68] and the confidence interval was between 0.3-1.36 (95% confidence).
Clinical manifestations of intoxication in these workers included clumsiness,
abdominal pain, lumbar pain and peripheral pain as main symptoms. The results
of this study indicate that copying rooms are at high risk for lead poisoning.

EVALUATION OF SERUM OPSONIC CAPACITY AGAINST BORDETELLA PERTUSSIS BY CHEMILUMINESCENCE
Serum opsonic capacity against B. pertussis was studied by using quantitative
chemiluminescence (CL), a method known to have several advantages over
conventional methods in evaluating opsonization and phagocytosis. Sera from
unvaccinated infants was shown not to contain opsonins against B. pertussis and
in unvaccinated infants suffering from whooping cough, no opsonins were
detected. In adults fully vaccinated during childhood, antibody titers decreased
with time. Therefore, antibody transfer to infants is negligible. The CL assay is
simple, rapid, and reproducible, offering new possibilities to evaluate humoral
immune mechanisms and phagocytosis in whooping cough.

RESPONSE OF METACARPAL FRACTURE FIXATION CONSTRUCTS TO PHYSIOLOGICAL LOADINGS
This study compares the functional capabilities of different metacarpal
fixation constructs under physiological loadings in an attempt to identify the
optimal construct rather than the strongest one. One hundred and twenty-six
preserved human metacarpals were mechanically tested after oblique osteotomies
and internal fixation. Maximum load to failure, average structural rigidity, and
energy absorbed were determined. All the fixations, except the intramedullary
rods, tolerated the assigned physiological loadings below their failure limits in
tension and torsion. The safety factor for K-wire tension band in bendings was only
1.4, which is very low compared to those of dorsal plate fixation (4.3) and the two
interfragmentary lag screw fixation (4.0). Both torsional and axial rigidity of the
K-wire tension band fixation were significantly less than the two interfragmentary
lag screw fixation. Fixation by two interfragmentary lag screws was the optimal
method, providing adequate strength and stability while requiring less soft tissue
dissection than dorsal plate fixation.

RELATIONSHIPS BETWEEN LIPOPHILIC CHARACTER AND BIOLOGICAL ACTIVITY OF NEW POTENTIAL LONG-ACTING LOCAL ANESTHETICS
The partition coefficients (P-values) of certain new potential, stereoselective,
reversible, long-acting local anesthetics have been determined in an n-octanol/
phosphate buffer system. These are derivatives of 2-phenoxyethyldialkylamine
hydrochloride, and almost all of them are readily soluble in water. Their aqueous
solutions have shown different absorption maxima, and these have been used
quantitatively to determine the concentration of solute in aqueous phase using the
Beer-Lambert equation and the calibration curve, which has been a straight line
within the test concentrations. The values of P have been calculated as the ratio of
the concentration of solute in the octanol phase divided by the concentration in the
aqueous phase the former value has been obtained from mass balance. The mass
balance is confirmed by obtaining the absorption measurements of the organic
phase. The guinea pig intradermal wheal test has been used to determine the
anesthetic properties of the test agents. They have exhibited better anesthetic
profiles than those of the bupivacaine standard. Moreover, they have displayed no
apparent side-effects neither locally nor systemically. Although there has been no
sharp general correlation between the P-values and duration of action (WT), it is
observed, in many cases, that derivatives with low P-values have shorter WT than
those with high P-values. Therefore, it seems that the duration of action of these
compounds is not influenced only by this property, i.e. P-values, and other factors
such as physicochemical, pharmacokinetic, and pharmacodynamic parameters as
well as non-specific binding characteristics at adjacent tissues are also involved.
Thus, the action of these new compounds is due to the molecule as a whole and not
to any one particular physical or chemical property. Furthermore, these are
reversible and stereoselective agents.

DOWN\'S SYNDROME: CYTOGENETIC STUDIES IN 150 CASES IN TEHRAN
Cytogenetic studies were performed on 150 cases of Down's syndrome (DS)
in Iran. The standard trisomy 21 was found in 132 (88 % ) and translocation-trisomy
21 (+21) in 18 (12%) patients, i.e., t(21,21) in 1(0.63%) and mosaicism in
17(11.33%) cases.
The comparison of the frequencies for mosaicism between different
populations such as Denmark, Hungary, Egypt, Iraq, India, Australia and Iran
demonstrated a difference in geographic distribution. There was a high incidence
in the north of Europe towards Egypt and Iraq which decreased towards Iran and
further towards the eastern region in the Indian ocean and India and further
decreased towards Australia. Statistical analyses demonstrated significant
differences between the data in Iran and Copenhagen, Hungary and Australia for
mosaicism and translocation +21, and India, for translocation +21. The occurrence
of translocation +21 decreased significantly from Denmark towards Egypt in
Africa and Iraq in southwest Asia, then it increased from Iran towards Australia
in the Pacific ocean.
The comparison of cells having satellite associations (SA), significantly
indicated the involvement of two and three SAs in DS cases.
The study on the position of chromosomes in the metaphase plate, the
occurrence of chromatid breaks and endoreduplication did not present any
significance in DS cases.

CYTOGENETIC BIOMONITORING OF 65 RADIOLOGY TECHNOLOGISTS OCCUPATIONALLY EXPOSED TO CHRONIC DOSES OF X-IRRADIATION IN IRAN
The frequency of chromosomal aberrations was studied in the peripheral blood
lymphocytes of 65 radiology technologists CRT) working at hospitals chronically
exposed to x-rays. Although film dosimetry did not show the maximal annual
permitted dose in any of the examined subjects, cytogenetic analysis detected fairly
high levels of chromosomal aberrations in R T compared to unexposed controls. The
mean frequencies of structural chromosome aberration per 100 lymphocyte metaphases
of workers and the controls were 2.93 and 0.54 respectively, excluding the high level
of achromatic lesions registered. The difference between them was statistically
significant with a P-value of <0.05.

HLA CLASS I IN IRANIAN P SORIATIC PATIENTS
The association of HLA and disease varies in different populations. We have
. studied the association of HLA class I in Iranian psoriatic patients, in order to
compare this association with other reports. Fifty-one Iranian patients with
psoriasis were HLA typed. The frequency of HLA antigens in patients and patient
subgroups, based on clinical patterns, age of onset, family history and provocative
factors, were compared with each other and with normal controls. The results
indicate that the disease is strongly associated with HLA-Cw6 (chi-square,
corrected P-value (Pc)≤0.0009), and negatively associated with Cw3 (Pc≤
0.00009 and Pc≤ 0.03, respectively). Psoriatic patients with early onset disease
and non-pustular psoriasis (NPP) showed a more significant association with
HLA-Cw6 (Pc≤ 0.0004 and Pc≤ 0.0003, respectively). There was no statistically
significant relationship between the presence of a positive family history and early
onset of disease.

TUBERCULIN SENSITIVITY IN AN URBAN POPULATION IN IRAN
Mantoux test using 2 IU of tuberculin prepared in Iran was performed on 6675
healthy persons randomly selected from an urban population (Shiraz) of Iran.
In 3776 persons with no evidence of previous BCG vaccination the rate of
positive reaction was less than 2% up to the age of 14, slightly more than 2% among
those 15-19 years of age, but strikingly higher (45.5%) in older age groups (P<0.001).
The rate of positive reaction was higher among females (P90%) again. The differences between different age groups as well as
the two sexes were statistically significant (P<0.005), though the sex difference was
reversed in this group.

SURV EY OF THE PREVALENCE RATE OF BEHAVIORAL DISORDERS AMONG THALASSEMI C PATIENTS IN ISFAHAN
Chronic diseases have been identified as predisposing factors in behavioral
disorders and the prevalence of these disorders is known to be higher in patients
than in control groups. Thalassemia too, plays an effective role in increasing the
prevalence of behavioral disorders.
In order to recognize the prevalence of behavioral disorders in thalassemic
patients, a study was performed on 257 patients (144 boys and 113 girls). A control
group was considered for comparison, the members of which were chosen from
healthy sisters and brothers of the patients, otherwise from their fIrst class family
members, and ultimately from their neighbours. The diagnosis of behavioral
disorders was based on DSM-III-R and the required information was collected by
questionnaires that were filled in by the parents and by interviewing the patients
themselves. The following results were obtained in the final inspection: 1) the
prevalence of separation anxiety (P = 0.000), enuresis (P = 0.021) and depression
(P = 0.002) was higher in thalassemic patients, and 2) other disorders under study
(oppositional disorders, attention deficit disorder with hyperactivity, conduct
disorder, avoidance disorder,- over-anxious disorder, pica, encopresis, stuttering,
elective mutism and stereotyped-habitual behaviors) did not show a significant
difference regarding a P-value above 0.05.

PAUWELS\' OSTEOTOMY FOR FEMORAL NECK NON-:union:
During a period of nine years (1982-1991), Pauwels' osteotomy was performed
for 38 cases of femoral neck non-:::union:::. 32 led to consolidation with one osteotomy
(84%) and 6 failed (3 needed revision (92%), one died and two were lost to follow-up).
The length of follow-up was 2-11 years, the average time of non-:::union::: 12
months (ranging from 4 months to 13 years) and the mean patient age 33 years
(ranging from 3-58 years).
According to the method of fixation the cases were divided into 3 groups, and
the best results were achieved in group III which were treated via a modification
of Pauwels' technique.

EXPERIENCE WITH 115 PATIENTS WITH CONGENITAL A DREN AL HYPERPLASIA AND EVALUATION OF GROWTH PATTERNS IN 24 PATIENTS WITH THE SALT-LOSING TYPE
In this study the data on 115 cases of congenital adrenal hyperplasia (CAH)
who were followed in the Pediatric Endocrine Clinic at Nemazee Hospital, Shiraz
will be reported.
Among these cases 51 were male and 64 female. The most common type of
CAH in these patients was the salt-losing type of 21 -hydroxylase deficiency
(85 .2%). ll-hydroxylase deficiency was present in 13.04% of patients. There was
only one case with 20 ,22 -desmolase deficiency and one also with 3-betahydroxysteroid
dehydrogenase deficiency.
Presenting complaints were ambiguous genitalia, vomiting, failure to thrive,
precocious puberty and hypertension.
The analysis of data on 24 patients with the salt-losing type of21-hydroxylase
deficiency who were followed for at least 2 years showed that these patients
suffered from abnormal growth patterns. Growth failure was maximal during the
first year of life.

OMENTAL GRAFT APPLICABILITY IN EXPERIMENTALLY INDUCED SPINAL CORD COMPRESSION IN RATS
One of the most important factors responsible for axonal degeneration
following spinal cord trauma is ischemia produced by cord compression.
Previous studies have revealed that omental transposition upon the injured site of
the spinal cord could be beneficial in the induction of partial improvement of
neuroelectrical and motor function in laboratory animals. The purpose of this
study is to verify the effects of early placement of pedicled omentum on recently
traumatized rat spinal cord as measured by subsequent clinical, electromyography
(EMG), motor evoked potential (MEP) and horse-radish peroxidase (HRP)
labelling methods.
Forty rats were divided into two groups (A;B). Atter anesthesia,
laminectomies were performed at T12level and the left lateral half of.the spinal
cord was compressed intradurally by an aneurysm clip for seven minutes in both
groups. The omentum was transposed in group B rats on the lesioned site.
After 3-5 months, evaluation of the muscular forces in the affected limb
according to modified Tarlov's classification showed a significant difference
between the two groups (P < 0.01). EMG showed a greater degree of spasticity in
nongrafted animals than grafted ones. MEP showed a powerful signal in grafted
animals, but only a weak signal was recorded in the nongrafted group. Injection
of HRP into the spinal cord below the compression site demonstrated retrogradely
labelle d neurons above the compression site in grafted animals, but transmission
of HRP was not seen in nongrafted animals.
These results show that transposition of omentum on the injured site of
recently traumatized rat spinal cord could be helpful in axonal regeneration.

KIDNEY TRANSPLANTATION IN CHILDREN AND ADOLESCENTS OF SOUTHERN IRAN
In this first report on the experience with renal transplantation in children and
adolescents in southern Iran during the past five years, thirty-two patients (aged 5-
19 years old 18 males, 14 females) received renal allografts. Pretransplantation
pathologic diagnoses by kidney biopsy were: membranoproliferative
glomerulonephritis (MPGN), rapidly progressive glomerulonephritis (RPGN),
chronic glomerulonephritis, chronic pyelonephritis, reflux nephropathy,
obstructive uropathy and renal tubular acidosis (RTA). The cause of ESRD was
unknown in eleven cases. Overall one year graft survival was 89%, 71 % at two
years, and 50% at four years. The overall patient survival at one year was 93% and
at two years, 86%. Well-being and remarkable improvement of physical,
emotional, biochemical and endocrinological impairment were noticeable in 74%
of the patients. No recurrence of primary disease was detected during the study
period.

CEREBRAL HYDATIDOSIS: A REAPPRAISAL
The remarkable evolution of surgical management of cerebral hydatidosis
towards an earlier diagnosis and proper treatment is evident from this 22-year
retrospective study of our experience with 19 cases seen at Shiraz University
Medical Institutions. Both cases of iatrogenically-induced infected cysts were
seen during the pre-CT era. The optimistic view of being able to remove all the
cysts intact seems to be elusive because six cysts ruptured during extraction even
with application of the Dowling technique. These cases were followed for a mean
of 44 months and in only one case was the patient referred again with subarachnoid
cysts of the lumbosacral region after two years. In six cases we had involvement
of other organs also. There were two cases of multiple cysts in the brain, a case with
de novo infection of the cyst contents, and one in which the cyst was epidural in
the parieto-occipital region. One patient died pre-operatively and three were lost
to follow-up however, fifteen patients were followed for a mean of27 months with
eleven being neurologically intact and four with focal neurological deficits. One
patient with a tapped cyst before extraction was readmitted two years later with
paraplegia due to drop parasitic infestation of the thoracolumbar region.
